What is a facelift?
Facelift surgery, also known as rhytidectomy, is a plastic surgery procedure performed to improve the neck laxity, jowls, and the midface. As a person ages, the midface begins to descend, producing the nasolabial folds. The lower face then begins to show jowling, near the chin and the neck muscles sag, producing banding and redundancy. These three areas can be improved with the facelift procedure. The facelift does not improve the skin texture or quality.


Who is the best candidate for a facelift?
The best candidate is a healthy man or woman whose face is beginning to show signs of facial aging. The typical patient may have sagging skin in the neck and jowls. He or she may also have midface descent, producing a nasolabial fold. There are many other factors that determine whether an individual is a good candidate for a facelift, such as skin type, ethnicity, and skin laxity. The surgeon does not put a strong emphasis on the age of the patient, but instead focuses on the patient’s facial aging.



Are there any limitations to what a facelift can achieve?
The facelift procedure cannot stop the aging process. It, however, make the person look more refreshed and rested. It is commonly said that a facelift will make the person look ten years younger.



What happens during a consultation for a facelift?
The surgeon will personally meet with you during the consultation. He will examine and evaluate you. He will then show you before and after pictures and then explain the surgical procedure from beginning to end. This explanation will include what surgical technique he will use, where the surgery will be performed, what type of anesthesia, and a step-by-step explanation of what the recovery will be like. You will then meet with the patient care coordinator, who will explain the scheduling details and the cost.



Where is facelift surgery performed?
The surgeon performs the facelift procedure in his clinic and affiliated hospital under local anesthesia with sedation.



What type of anesthesia is used for a facelift?
The surgeon will perform the facelift with either local anesthesia and sedation or general anesthesia. If general anesthesia is used, it will be administered by an anesthesiologist (MD). The surgeon will discuss which option is best for you.



How is the actual facelift procedure performed?
The surgeon will perform a specific type of facelift, the deep plane facelift. This facelift technique has the advantage of producing a very natural result, while providing a very smooth neck contour. It also improves the jowls and the midface.
The procedure involves making a series of incisions. The neck muscles are tightened through an incision just under the chin. Then in the lower face, the facial muscles are identified and repositioned in their new location. The skin is then re-draped and the excess is trimmed. All of the incisions are closed with very tiny stitches so that they heal without scars. At the conclusion, a dressing is then placed.



How long is the surgical time for a facelift?
The surgery will take about three hours to perform a facelift.



Where are the incisions made for a facelift?
The surgeon places all the incisions so that they are all hidden and out of site. The incision in the temporal area is hidden inside the hair. In front of the ear, it loops behind the tragus (the small cartilage in front of the ear). Next it travels behind the ear in the crease and then into the hair.



Are there scars that result from a facelift?
The scars from a facelift should not be visible after they have healed. There are two reasons why facelift scars are visible. One, because the skin was closed with too much tension. The second is because the incision was placed in a poor location. The surgeon uses the deep plane technique to avoid any tension on the skin. He also places the incisions in locations where they will not be visible and uses meticulous techniques to close the incisions.



Is there much pain with a facelift?
The surgeon will prescribe you pain medication in case you need it. Most patients do not feel pain after the facelift procedure.



Will I need someone to help me out at home after the facelift procedure?
It's a good idea for patients to prearrange for post-surgery support from family members or friends.



What is the recovery like after a facelift?
The recovery for the facelift procedure is rather mild. The surgery is ambulatory, meaning that the patient is allowed to return home the same day of the surgery. The surgeon will see you in his clinic the morning after surgery. He will remove the dressing and check all of the incisions. He will then place a removable elastic bandage to help support the tissues during the healing phase. The patient will then return to the office on the fifth post-operative day. At that point, the small sutures around the ears and under the chin will be removed. On the tenth post-operative day, the sutures behind the ears will be removed. From here on the heeling will move forward quite rapidly. Bruising will gradually subside. By the two week mark the patient will be presentable. Most patients return to work at this time.



When will I be able to return to work after my facelift?
Patients are generally able to return to work about two weeks after the facelift procedure.



How soon after my facelift will I am able to exercise?
The surgeon recommends that his patients avoid strenuous exercise for three weeks following surgery.



How soon after the facelift procedure are the stitches removed?
The surgeon will remove the stitches around the ear and in the neck in the fifth post-operative day and the ones behind the ear on the tenth post-operative day.



What are the risks or complications associated with facelift surgery?
Some of the potential risks associated with facelift surgery may include infection, bleeding, scarring, nerve injury, and poor healing. These are very rare incurrence.


Are the results of a facelift permanent?
T
he results of a face lift may be considered permanent in that you will continue to look younger years later than you would have without undergoing the face lift. However, a facelift cannot stop the aging process and the face will continue to age. Many say that the face lift tends to last about five to ten years, after which time patients may opt to undergo a touch-up procedure.

When choosing a plastic surgeon, it is important to consider a number of factors. Medical training and surgical experience are primary indicators of the surgeon’s qualifications.  Board certification is another important criterion. No single factor can guarantee you the best doctor for a specific procedure. Ultimately, it is up to you to decide which surgeon is best for your specific goals and expectations.
To become a plastic surgeon, a doctor must obtain an M.D. (Doctor of Medicine) or a DO (Doctor of Osteopathy) degree. Following this, there are several paths the doctor may follow. Many doctors now spend five to six years in an integrated plastic surgery residency, although some complete three years of general surgery and two years in plastic surgery.

Choosing a Board certified surgeon can be a very tough job because of the personal information being shared. Just open the yellow pages, magazines, brochures or view news paper advertisements and you’ll find plastic surgeons promoting all kinds of society memberships and board certifications but don’t be confused between boards and societies. Societies are merely professional associations. Board certification is the minimum criterion you should use in selecting a plastic surgeon. So, it is very important that you learn about the doctor’s education, training, certification, and experience in order to guarantee a successful outcome and to make the best choice.

Beware of lots of Letters after the Doctor’s Name:
Many fraudulent doctors’ have quite a few letters after their names. In fact, multiple letters may be the telltale sign of an inadequately trained doctor trying to impress the public. Definitely, your doctor should have the letters M.D (Medical Doctor) or D.O (Doctor of Osteopathy) after his name. He might even have one or more master’s degrees or might also have PhD but these degrees are irrelevant to plastic surgery. Patients are advised to select a board certified plastic surgeon.

  • Get Referrals: Begin your search for a plastic surgeon by asking friends, relatives, family doctor, colleagues that are willing to meet with you and share their results of the surgery. Hospital staff such as nurses, surgeons and anesthesiologist is also excellent sources of referrals. You may also do research on the internet, magazines, or in the yellow pages. Chances are a few names will stand out from the others. It will help give you the best possible recommendations.
  • Check the qualifications of the surgeon: One of the most important qualifications you should look for in a plastic surgeon is that they are board certified but you must need to know what board has certified your plastic surgeon and be sure that the surgeon has graduated from an accredited medical school.
  • Meet the surgeon: Look at pictures of past procedures done by the surgeon. Choose an ethical and confident surgeon and friendly staff members for a long lasting relationship and you should expect good communication, understanding and comfort from the surgeon.

Temporal Lift

A temporal lift is a cosmetic surgery procedure designed to address signs of aging around the eye. This procedure can be combined with a forehead lift, eyebrow lift, and other cosmetic procedures.
The principle is to make a short incision ( long like the last finger) in the hair above the ear, parallel to the hair line but behind the anterior hairline, and to detach (undermine) in depth the anterior portion of the forehead and temple skin, eyebrow area and malar cheekbone skin areas. Then this area will be pulled and deep stitches will be placed to reposition. The incision and the scar are therefore hidden in the hair and not visible.
Duration o the operations:
The procedure usually takes is about 1 hour depending upon the extent of surgery.
Post operative care:
Recovery time and after-effects are simple and short and of various duration depending on individuals. Usually a week off social life is enough. The small clamps staples in the hair-bearing scalp are removed on the 8th day. Swelling of the upper part of the face and eyelids due to postoperative edema and ecchymosed will decrease gradually over the first 10 days, and then the residual edema will disappear in the next few months. It is usual the first weeks to have a bulge in front of the hairline: this will disappear spontaneously. Loss of hair is a rare event. Sensitivity and subtle motility of the scalp around the incision will return within a few months (with a possible pruriginous stage).
Recovery period:
We recommend only light activity for the first week to ten days following surgery. Most patients are able return to work 7-10 days following surgery with makeup easily covering any residual bruising.
Possible risk and complications:
Complications are very the unusual evolutions that require further treatment. The possible complications of the temporal lift are rare: Hemorrhaging, Hematomas and infection which may require an additional procedure and a specific medical therapy ,skin necrosis, and pain are very rare events, poor scaring, adherences, fibrous scar, visible scar, transitory paresis, or a paralysis of the frontalis or supercilii muscles  Impaired mimic or asymmetrical mimic, asymmetric eyebrows
Local alopecia, loos of hair local anaesthesia of the scalp, persistent asymmetry, itchiness
Psychological disorders (depression, psychosis), cardiovascular, immunoallergic or respiratory complications of anesthesia.

This procedure may be performed in the cosmetic surgeon’s office-based facility, an outpatient surgery center, or at a hospital. This procedure is usually done on an outpatient basis.
Preparation before Surgery
  • Assessment of the medical history (any allergies, serious medical condition and all medications taken both prescribed and non-prescribed), physical examination, and laboratory tests will be performed during consultation.
  • Blood and urine samples will be collected for routine preoperative laboratory tests.
  • Smoking must be avoided for about 3-4 weeks prior to surgery, as nicotine interferes with circulation and will greatly affect healing process.
  • You will likely to be asked to stop drinking alcohol, a week before the surgery and throughout your recovery period.
  • Avoid taking any medications such as hormones, anticoagulants, anabolic steroids and supplements at least 4-6 weeks to prevent complicating medical factors prior to surgery. Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding. 

Total Facelift

Total Facelift also known as rhytidectomy, is a cosmetic procedure to reduce sagging skin and lines by removing excess skin and fat, and tightening muscles 
The incision of full facelift will begin above the hairline at the temple area, brought in front of the ear and looping behind the ear lobe and along the lower scalp. In full face lift the neck and face will be tightened altogether. The exact placement of incisions is determined upon the actual examination of facial structure. The patient may opt to do chin liposuction to enhance the result.
Duration of the operation:
A full facelift should take anywhere from 3 – 7 hours, depending on the patients requirements
Post operative care:
After the surgery, a patient will have to use cold compress for 2 days and avoid lying with the cheeks in contact with bed or pillow for 1 month to prevent hematoma formation. Some numbness may persist for 3-6 months. The patient should avoid any strenuous activities for at least 4-6 weeks following surgery.  This will give the body ample time to adjust as it continues on the healing process.
Recovery period:
Facelift sutures are removed between 5-7 days after surgery, and at this point swelling and bruising are becoming minimal. Makeup can usually be applied, and patients will start to resume some of their activities.
Possible risk and complications:
Full facelift surgery is a very common procedure and complications are rare, but can happen. This includes prominent, red scars, bleeding under the skin, infection, hair loss surrounding the scars, nerve damage, numbness, asymmetrical features and blood clotting.

You may opt for either local or general anesthesia

Mid Facelift

Mid facelift is a plastic surgery procedure that restores a smooth, youthful look to the lower eyelids and cheeks. During a mid face lift, small, deep incisions are made in the muscle tissue of the face. If the surgeon so chooses, an endoscope (a small, camera-like device) is inserted through the incisions to view the procedure internally. Once the incisions are made, the doctor adjusts the fat and muscle tissue, pulling up the middle of the face to smooth and tighten the skin. Finally, the incisions are sutured.
Duration of the operation:
The midfacelift usually takes about 3 hours.
Post operative care:
Although discomfort should be minimal immediately following face lift surgery, patients will not be in a condition to drive home. Their head will be bandaged and they may still be experiencing the effects of anesthesia. They should therefore arrange for transportation to and from the treatment center with a friend or family member. During the week following face lift surgery, they should also plan on maintaining a relatively light schedule. Often, having someone to help with everyday activities such as cooking and cleaning can make the recovery period much easier.
Recovery period:
Most patients can return to work from a mid face lift in 10 days to two weeksFor the first few days or weeks, depending on your procedure, only walking and mild stretching are appropriate physical activities. Vigorous exercise, heavy housework, and sexual activity should be avoided for at least the initial two weeks after surgery. Alcohol, saunas, and steam rooms should be avoided for several months. During the face lift recovery period, extra rest is very important to promote proper healing.
Possible risks and complications:
Aside from the risks inherent in any surgical procedure, risks specific to face lift surgery include:
  • Injury to the nerves that control facial muscles - this is usually temporary.
  • Problems with healing of the skin - this is common for patients who smoke.
  • Scarring - there will be some scarring, although the surgeon will always try to hide the face lift scars.
  • Crust on the incisions - this will be temporary.
  • Numbness or tingling around the incision areas - this should be temporary.
  • Skin or hair loss - this is rare.
  • Skin discoloration - this is a rare condition that can continue for several months.
Some of the most common postoperative face lift complications are:
  • Reaction to the anesthesia – some face lift patients experience nausea and general sickness after receiving anesthesia; this is a short-lasting side effect and usually diminishes completely after a few hours.
  • Hematoma – if bleeding occurs under the skin, a hematoma can develop, leaving the area painfully swollen; an operation is required to stop the flow and remove the blood.
  • Bleeding – although some light, postoperative bleeding is normal, uncontrolled bleeding is dangerous and can lead to painful hematomas.
  • Infection – typically occurring in a specific area, symptoms of infection are swelling, sensitivity, redness, and localized heat. Unless it is severe, an infection can be relieved with antibiotics.
Anesthesia use:
Mid face lift surgery procedures can be performed using local anesthesia although general anesthesia is also an option. During more intensive procedures, general anesthesia may be required.
